Why is Firefox displaying so many of the not found images? I have dumped recent cache data a few times already and they still appear. These are the images that say "more osm coming soon".
Do you have an AdBlock plugin installed? I remember some people having the same problems as you because they accidentally put one of the tile servers into their blacklist.

Depends what the tile server is up to, they may be rebuilding the database on the OSM tile server so new tiles won't be created.

You might have accidentally blocked one of the three image servers:
a.tile.openstreetmap.org, b.tile ... etc. Check that none of these are blocked.
