Add missing to-member for turn restriction

Fix no_u_turn relation. So far there was only a single via-node member.

Do not use restriction:hgv and restriction at the same time. Discussed this with Lukas.

Fix and/or delete some broken turn restrictions.

Fix and/or delete some broken turn restrictions. They were missing from/via/to members and some were already covered by nearby restrictions anyway.

Fix restriction relations that use restriction *and* restriction:bicycle tags. The restriction tag already includes bicycles. In case a turn should *not* be restricted for bicycles use except=bicycle instead of bicycle=no (see OSM wiki).
