highway=residential should be used for (public) roads shared between a number of houses. highway=residential

Whether the road is shared / private is an important divider between the two.

In rural areas, and very rarely in urban areas, I would use the first option for up to one business / farm and two houses. Otherwise highway=residential or highway=unclassified

Essentially the little square was drawn twice, using the same nodes. I've noticed it happen if there is a problem when trying to save and it is saved again.
